Proof of Documents Required

Documents to be enclosed as to the correctness of Address and Age

Every application for issue of a driving license whether Learner or Permanent, or authorization to drive a Transport Vehicle or a Renewal of Driving License or issue of a Duplicate Driving License wherever there is a change of address or change of address in the Driving Licenses shall have one or more of the following documents in original or relevant extracts thereof duly attested by a Gazetted Officer of the Central Government / State Government or an officer of a local body who is equivalent in rank of a Gazetted Officer of the Government to establish the identity of the applicant.

• Aadhaar Card
• Ration Card
• Multi Purpose House Hold Card
• Pay slip issued by Public Sector Organization showing his office address / residential address
• Municipal Tax notice / Receipt
• Bank Pass Book from any Public Sector / Private Sector / Banking Institution included in the Second Schedule to the Reserve Bank of India Act, 1934.
• Cooking Gas Connection allotment order
• Electricity Bill issued by the DISCOMS
• Water Bill issued by Municipality or Water Works Department
• Any Telephone Bill including Land line or Mobile
• A Driving License or Registration Certificate issued by the concerned Licensing Authority or Registering Authority as the case may be.
• Aadhaar Card,
• Electoral Roll,
• Life Insurance Policy,
• Passport,
• School Certificate,
• Birth Certificate,
• Certificate granted by a registered medical practitioner not below the rank of a Civil Surgeon, as to the age of the applicant.
